Admission Criteria for Memory Lane
Memory Lane exists to provide residents with a cognitive or Alzheimer’s/Dementia related diagnosis a safe and comfortable environment where staff has been trained to care for their specific needs. Memory Lane is more than a Dementia Unit. Residents must manifest symptoms of cognitive impairment including disorientation, short and long term memory impairment, communication deficits, impaired attention span, impaired judgment, personality changes, mood swings, agitation, and irritability or wandering.
Residents may also be admitted to the Program based on the following guidelines:
- Resident has the ability to function in group dynamic.
- Resident is able to actively or passively participate in individual or group programming.
- Resident is medically stable.
All residents are evaluated prior to admission and upon a significant change. Admission is determined by an evaluation which includes but is not exclusive to; assessment of functional abilities, mental status, and psychosocial care needs, behavioral issues, diagnoses, medications/treatments, and medical history.
Admission is determined through an interdisciplinary review process on a case by case basis and is subject to the Facility’s determination of ability to meet the resident’s needs.
